Authorization Form

Authorization forms for patients can be completed by any Affordify platform user.

*Requirements:

Google Chrome Browser (can be installed from https://www.google.com/chrome/)
Stable internet connection (high-speed preferred and recommended)

Creating a new patient in Affordify

Sign in from https://affordify.net
Search for an existing patient or click on the “Checkout New Patient."
In the upper right corner of the Patient Information section, you will have two options to complete an authorization form if one has not been completed already.

Sign Authorization form - open a window with authorization form to be completed on the spot.
Email Auth form - email a link to the patient to complete authorization form.

Once the Authorization form is completed, you will see the Download Auth form icon appear in the upper right corner of the Patient Information section

In the upper right corner of the Patient Information section, you will notice a different number of icons, depending on what the patient has on file.

Patients who DO NOT have an auth form on file, and DO NOT have an email will see these icons.
